Most sore throats are caused by viruses and clear up on their own within a week. But some are bacterial and benefit from antibiotics. Under the NHS Pharmacy First scheme, we can assess your symptoms and decide which is which.

Working through Selly Oak high street and feeling rough? You can walk into Selly Pharmacy at lunch, get seen, and be back at your desk within half an hour. We see a lot of University of Birmingham students and local residents through the autumn and winter months.

Who is eligible?

The NHS Pharmacy First sore throat service is available to:

  • Adults and children aged 5 years and over.
  • People who are not pregnant (see note below if you are).
  • People who do not have signs of more serious infection requiring urgent care.

What we will do

The consultation takes about 15 minutes in our private room. We will:

  • Take your temperature and check for swollen lymph nodes.
  • Look at the back of your throat for pus or white spots on the tonsils.
  • Use the FeverPAIN clinical score to estimate the likelihood of bacterial infection.
  • Either reassure you and recommend self-care, or prescribe a course of antibiotics on the spot.

Self-care advice

If your sore throat is viral (most are), the best things you can do are:

  • Drink plenty of fluids and rest.
  • Take paracetamol or ibuprofen to relieve the pain.
  • Use medicated lozenges or sprays containing local anaesthetic.
  • Gargle with warm salty water (not for young children).

When to seek urgent care instead

Please go to A&E or call 999 — not the pharmacy — if you have:

  • Difficulty breathing or swallowing.
  • Drooling (in a child).
  • A muffled voice or stridor (high-pitched noise on breathing in).
  • A rash that does not fade when pressed with a glass.

If you are pregnant

We can still see you and give you advice, but the prescribing element of Pharmacy First is more cautious during pregnancy. We will likely refer you to your GP or midwife for a prescription if antibiotics are needed.

Will I always get antibiotics for a sore throat?
No, and you should not expect to. Most sore throats are caused by viruses, where antibiotics do nothing and can do harm. We use the NICE FeverPAIN scoring system to identify the small number of patients who genuinely need antibiotics.
How does the FeverPAIN score work?
FeverPAIN looks at five things: fever in the past 24 hours, attendance within 3 days of onset, severe tonsil inflammation, pus on the tonsils, and absence of cough or runny nose. A higher score increases the chance that antibiotics will help.
Can children use Pharmacy First for sore throat?
Yes. The NHS Pharmacy First sore throat service is available for adults and children aged 5 and over.
What if my child is under 5?
Children under 5 with a sore throat should be seen by a GP or, if very unwell, taken to A&E or NHS 111. We cannot assess them under Pharmacy First.
How long does the consultation take?
Around 10 to 15 minutes. We check your symptoms, look at your throat, and explain the FeverPAIN result on the spot.
Is the consultation really free?
Yes. The consultation is free under the NHS. If antibiotics are prescribed and you pay for prescriptions, the standard NHS charge applies.
Should I just see my GP?
For most sore throats, the pharmacy is faster, free, and follows the same NICE guidance your GP would. If your symptoms are severe, persistent, or you are not eligible, we will refer you on.
